Once your reservation is made, please submit either a hard copy or the online form of the following items to help facilitate the check in process:

• Pet Information Sheet for each individual pet

• Health Care Certificate from your veterinarian indicating your pet is current on his or her vaccinations. The certificate may be faxed, e-mailed or mailed to us but we must have it prior to your pets stay with us.

Vaccinations required for dogs:

• Rabies
• DHLLP
• Bordetella (kennel cough)

Vaccinations required for cats:

• Rabies
• Feline Distemper
• Rhinotrachetis

I strongly recommend Frontline flea and tick control be applied prior to boarding. For the protection of all our guests, a pre-entry exam will be given to all pets. If a pet is found to have fleas and/or ticks we will treat your pet with Frontline at the owner’s expense.
